Insurance Reimbursement

Dr. Dorn is out of network with all insurance companies. You are responsible for payment at the time of each appointment. We require that patients place a credit card on file with us, through our secure platform.

If you have out-of-network mental health benefits, you may choose to file claims with your insurance company for partial reimbursement. You will be provided with a receipt at the time of service that contains the requisite information to submit a claim directly to your insurance carrier.

If you do not have insurance or do not plan on using your out-of-network benefits, you have the right to receive a Good Faith Estimate.

Dr. Dorn has opted-out of Medicare. If you have Medicare and wish to see Dr. Dorn, you are required to sign a written agreement that states that you will not submit claims to Medicare for reimbursement.

Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

Under the law, health care providers need to give patients who do not have insurance, or who are not using insurance, a cost estimate of the bill for medical items and services.

  • You have the right to receive a ‘Good Faith Estimate’ for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • For questions or more information about your right to a Good Faith Estimate, visit http://www.cms.gov/nosurprises
